New patients forms, please print and fill out the following forms and bring them to your first appointment.
Forms provided below are to help make your first visit to be as care and stress free as possible. When coming into the office please bring your drivers license, insurance card and completed new patient forms to help ease your first visit. Dr. Anthony Adams really wants to know you and takes the time to go over your past and present concerns. So the more complete your patient forms are, will allow Dr. Adams to get a better understanding of your dental history. Dr. Adams is a general dentist with a more natural look to general and cosmetic dentistry, with a less advisee approach to achieve your over-all oral health. Dr. Adams believes in a mercury free office and is moving his practice to be mercury safe.
Please call the office if you have any questions or you need us to mail you out a new patient form packet. You can also come about 15minutes early & we will be glad to help you out with your new patient forms.
